Choosing the Appropriate Questionnaire

Finding out if Julian Gray Associates can help you and your family is as easy as completing one of our planning questionnaires. In order to properly and efficiently assess your situation, we ask all prospective clients to complete a confidential questionnaire. There are various versions of our questionnaires, with each version relating to a specific set of circumstances, all of which are described with a brief summary below.

You may return the completed questionnaire to our office via fax, email, regular mail or by stopping by our office in Green Tree. You may also provide us with current copies of your estate planning documents which we will review in addition to your questionnaire, free of charge. Once we’ve reviewed the information you have provided, you will be contacted by our staff promptly to discuss your case and schedule an initial consultation.

Eldercare Planning Questionnaires

The Eldercare Planning Questionnaire is appropriate for people who are retired and beginning to experience some health concerns, or are recovering from a significant health event. These individuals may be residing at home or in a facility setting ranging from skilled nursing, assisted living, personal care home or independent living facility for medical treatment or assistance with activities of daily living. This questionnaire helps to identify long term care planning options, asset protection planning, probate avoidance and public benefits planning such as Veterans Benefits and Medicaid eligibility.

Retirement Planning Questionnaires

This questionnaire is appropriate for people who are currently retired or planning to retire soon. Generally these individuals are in good overall health and typically reside independently. Retirement planning is geared towards “empty nesters” and others whose lives have changed since the last time they prepared an estate plan. Retirees are looking towards securing their future golden years while protecting assets for future generations.
